Configuration:
1. yum install sipxcustomcallerid
2. service sipxconfig restart
3. in Features > Custom Caller ID, Settings tab enable custom caller id

4. In DNIS tab upload csv file with wanted rules
CSV file should have following format:
from,to
200,TEST1
201,TEST2
202,TEST3
"from" column contains extension to be rewritten and "to" column the value to rewrite caller id with
When placing calls from one of defined extension the calee will see the defined value instead caller's id
